Prince Sultan Military College of Health Sciences grades courses on a 5.00-point scale under the Council of University Affairs' unified "Study and Examination Regulations for the University Level" and the college's own Executive Rules, in force from the start of the 1444H academic year.

Beyond the letter grades that count toward the GPA, the academic record also uses several special course-status symbols that carry no grade-point weight and are excluded from the GPA: In-Progress (IP), Incomplete (IC), Denied (DN), No-grade Pass (NP), No-grade Fail (NF), Withdrawn-with-excuse (W), and Exemption (E).

The semester GPA is the total grade points earned divided by the total credit units of all courses studied that semester; grade points for a course are its credit units multiplied by the point weight of the grade earned.

The cumulative GPA is the same calculation applied to every course studied since the student joined the college.

A student receives an academic warning when their cumulative GPA drops below 2.00 out of 5.00 in any semester; a second warning follows if it stays below 2.00. A bachelor's student is dismissed after at most three consecutive warnings, though the college council may grant a fourth chance to raise the GPA by studying available courses.

First-year students follow a stricter track: dismissal after just two consecutive warnings for a cumulative GPA under 2.00/5.00, after failing any first-year course twice, or for a cumulative GPA still under 2.00/5.00 at the end of the first-year program. A student is also dismissed after 4 consecutive weeks of unexcused absence, or for not completing graduation requirements within one and a half times the standard program duration (extension provisions apply in limited cases).

At graduation, a student's cumulative GPA is classified as follows:

• Excellent: 4.50–5.00
• Very Good: 3.75–4.50
• Good: 2.75–3.75
• Pass: 2.00–2.75

A cumulative GPA below 2.00 out of 5.00 triggers the academic-warning and eventual-dismissal process rather than graduation.

First-class honors requires a cumulative GPA from 4.75 up to 5.00 out of 5.00 at graduation; second-class honors requires from 4.25 up to (but less than) 4.75 out of 5.00.

Both require the student to have never failed a course — at this college or another university, including as a visiting student — to have completed graduation requirements within no more than the average of the college's minimum and maximum allowed study duration, and to have studied at least 60% of graduation requirements at this college.

A student transferring in from outside the college generally needs a cumulative GPA of at least 3.00 out of 5.00 (female students may transfer at 2.50/5.00 given marriage, divorce, a relocated guardian, or a guardian's death). Transferring between specializations inside the college requires a cumulative GPA of at least 2.00/5.00 for a diploma-level move or 3.00/5.00 for a bachelor's-level move.

An external course is equated only where it has at least 80% content overlap with the college's own course, the student earned at least a "C" grade (or 70%) in it, and the student still completes at least 60% of graduation requirements at the college itself. Equated courses count toward graduation hours but are excluded from the semester and cumulative GPA.
